If your child requires school staff to administer medication during school time, we require one of the below forms to be completed and signed and sent to the office with the medication.
If you wish to discuss any information regarding medication please do not hesitate to either ring the school office on 9205 5800 or call into the office to discuss.
If an emergency action plan is required for specific conditions, we will require to meet with you to discuss completing this paperwork.
Please Note: Long term administration of medication should be incorporated in a health care plan and we can discuss this with you.
Medical Forms
It is important that we have up to date information regarding your child’s medical conditions. On enrolment, you are asked to provide your child’s health information to help the school meet their needs.
If your child has been diagnosed with a medical condition or if there have been any changes to an existing condition, please complete a medical form and return it to the office.
Medication at School
If your child requires medication to be administered at school, a separate form is also available.
Please note that no medication can be given without written permission, and all medication must be in its original packaging, clearly marked with your child’s name.
